At this surgery we have a Social Prescriber and a Health and Wellbeing Coach.

Healthtrainers are part of the YOURhealth team and are people just like you. They have been recruited from the community, bringing with them a range of skills, experience and local knowledge providing you with multiple services including

Weight Loss - Looking to lose weight in a healthy and manageable way, then the team can support you with personal goals that are achievable and specific to your lifestyle.

Healthy Eating -  The Healthtrainer team can help improve your diet, whether that be increasing your fruit and veg or reducing those unhealthy snacks.

Alcohol - We offer support to those who wish to reduce, limit and control their alcohol levels through innovative ideas and methods.

Emotional Wellbeing - Need someone to talk to? Having issues with your mood or the way you are feeling? The Healthtrainer team will listen and help you with improving your emotional well-being

Physical Activity - Looking to get active? The Healthtrainer team can help in removing barriers and finding new, fun and interesting ways of enjoying activity.

Healthtrainers come from a variety of backgrounds. They have the knowledge and experience of living and working in the community and they understand how difficult it can be to make healthy choices sometimes.

Healthtrainers have undergone extensive training and they keep up to date with all the latest health information. They will be out and about at public events to give out information and support you.

Healthtrainers will help you set achievable goals, develop your personal plan, identify and overcome barriers and provide ongoing encouragement and support. They know the local area and can put you in touch with local services, community groups, support and self-help groups.

How do I access the service?It is simple, you can contact the surgery and ask to speak to your Social Prescriber.
